About Grace Cottage Family Health
Grace Cottage has been certified by the National Committee of Quality Assurance as a Level 3 Patient-Centered Medical Home. A “Patient Centered Medical Home” (PCMH) is a way of coordinating primary care so that you get the right care at the right time. Your care will be coordinated by a dedicated team of health professionals all working together to meet your individual health care needs. Your primary care provider will be in charge of your team. Depending upon your individual situation, your team might include a registered nurse, a health coach, or a diabetes educator. Your team members will meet with you individually to help you set goals for better health and to support you in your efforts to meet these goals. We want you to feel “at home” with your health care at Grace Cottage.
