Requesting Your Medical Record
To request a copy of your medical record, print out and complete the Disclosure of Protected Health Information. Mail the completed form to:

Requesting “Highly Sensitive” Medical Records
To request a copy of medical records regarding your care and treatment for medical conditions of a highly sensitive nature (such as mental health records or alcohol treatment), print out the Behavioral Health Records Authorization and mail or fax to the Medical Records Department.
You also may call the Medical Records Department at (802) 365-7920 and request a copy of the appropriate form. Please allow 7-10 business days for us to respond to your request.
To Request X-Ray or Other Images
You may request a copy of your CT scan, X-ray, or other images by contacting the Diagnostic Imaging Department at (802) 365-3625. If you also need a copy of the radiologist’s report, please inform us when you call.
