If you have been prescribed an anticoagulation medication (“blood thinner”) and your primary care provider is part of Grace Cottage Family Health, you will be referred to our Warfarin (Anticoagulation) Clinic.
Anticoagulant medications reduce the body’s ability to form blood clots. The most common anticoagulant is called warfarin (also known by the brand names Coumadin or Jantoven). While these medications are very effective in preventing stroke and other complications, careful monitoring is necessary to keep patients safe from the risk of bleeding. The Grace Cottage Warfarin Clinic provides management of warfarin therapy on an outpatient basis for Grace Cottage Family Health patients.
The Grace Cottage Warfarin Clinic is staffed by pharmacists who work closely with your Grace Cottage primary care provider (PCP) to manage your warfarin therapy. Working with your PCP, the staff will check your blood tests and adjust your warfarin dosage as needed within the range prescribed by your PCP.
