You can:
- Review the information in your medical records. (See your records via the Patient Portal here.)
- Request a copy of your medical records. This may involve a fee.
- Request that your medical records be released to someone else.
- Dispute information in your record.

Requesting Your Medical Record
To request a copy of your medical record, print out and complete the Disclosure of Protected Health Information. Mail the completed form to:

To Request X-Ray or Other Images
You may request a copy of your CT scan, X-ray, or other images by contacting the Diagnostic Imaging Department at (802) 365-3625. If you also need a copy of the radiologist’s report, please inform us when you call.
