Where to Go If You Need Care Right Away?

By Keanna Cook, MSN, RN, ED Nursing Director, and Paul Stanchfield, PA-C, Same-Day Care Provider

When you or someone you care for suddenly doesn’t feel well, and needs urgent medical advice, the first question is often, “Where should we go?”

In stressful moments, it can be hard to determine the right level of care. At Grace Cottage Family Health & Hospital, we want our community to understand the differences between urgent care services available within the primary care office and care given in the Emergency Department (ED). Taking a few minutes now to understand the differences can make a big difference later.

Grace Cottage’s primary care practice provides same-day visits for many acute concerns, including minor injuries, infections, asthma or COPD flare-ups, urinary symptoms, rashes, or gastrointestinal illness – things that need help right away but are not life-threatening conditions.

Grace Cottage’s same-day clinic care visits are currently only available to already established patients. Because of that, we know your medical history, medications, and chronic conditions, and we can often provide more personalized and coordinated care than a stand-alone urgent care clinic. Follow-up is seamless, and care plans are integrated into your ongoing health management.

Stand-alone urgent care centers offer convenient extended hours and walk-in access to anyone, which can be helpful when primary care offices are closed. However, they may not have access to your full medical record, so tests may be duplicated, and continuity of care can be limited. Additional follow-up with your primary provider is often still needed.

For many non-emergency but urgent concerns, contacting your primary care office first is the most efficient and coordinated option. If you are unsure where to seek care, please call our office (802-365-4331) and we will help guide you. Our goal is to provide timely access while maintaining high-quality, relationship-based care for our patients.

When should you go straight to the Emergency Department (ED)?

If something feels seriously wrong and you want to be seen immediately, don’t worry about second-guessing yourself. When you walk through the doors of an ED, a team is ready for you. That’s exactly what an ED is designed to provide – immediate care for the unexpected, the severe, and the life-threatening.

The Grace Cottage Hospital ED is open 24 hours a day, 7 days a week – including weekends and holidays. Emergencies don’t wait for business hours. Whether it’s 2:00 in the afternoon or 2:00 in the morning, the ED is prepared to respond.

By federal law – the Emergency Medical Treatment and Labor Act (EMTALA) – every person who comes to the ED must receive a medical screening exam and stabilizing treatment, regardless of insurance status or ability to pay. That means if you are worried about your health, you will be evaluated.

So when should you choose the Emergency Department?

Come to the ED immediately or call 9-1-1 if you experience:

  • Chest pain, pressure, or tightness
  • Sudden shortness of breath
  • Signs of a stroke (facial drooping, arm weakness, slurred speech)
  • Severe bleeding
  • Head injury with confusion or vomiting
  • Severe abdominal pain
  • Seizures
  • Serious burns
  • Major injuries or trauma
  • A mental health crisis or thoughts of self-harm

These are not “wait and see” symptoms. These are moments when minutes matter.

A helpful rule of thumb is this: if symptoms are sudden, severe, worsening, or frightening, choose the ED. It’s also important to trust your instincts. You know your body best. If something feels seriously wrong, don’t hesitate to seek emergency care. It’s always better to be evaluated and reassured than to delay care for a potentially serious condition.

Grace Cottage’s ED is built for high-risk and high- stress situations, with advanced diagnostic tools such as CT scanners, x-ray, ultrasound, cardiac monitors, emergency medications, and rapid laboratory tests. If a patient needs treatment not available at Grace Cottage, the ED providers will stabilize you and then arrange for emergency transport by ambulance or helicopter to a larger hospital.

ED patients are assessed using a process called triage. When you arrive and check in, a nurse performs a quick initial assessment of your condition, which includes taking vital signs to determine how urgent your situation is. Patients are seen based on severity, not arrival time. That means someone who arrives after you may be taken back first if they are having a stroke, heart attack, or another life-threatening emergency.

Grace Cottage consistently maintains some of the shortest ED wait times in southern Vermont. While this can vary depending on the number and severity of patients being treated, the team works hard to evaluate patients as quickly and safely as possible.

No one plans for an emergency. But when one happens, you deserve rapid assessment, skilled clinicians, advanced equipment, and a team ready to act. That’s what the Grace Cottage ED is here to provide – around the clock, every day of the year.
