REGISTERED DIETITIAN – Per Diem

Job Summary:  Plans and delivers nutritional care to patients according to the physicians’ diagnoses.  Performs as a member of the multidisciplinary care team.  Screens newly admitted patients to identify those at nutritional risk.  Monitors and advises dietary employees and other support personnel.  Provides outpatient nutritional counseling to individuals as referred/ordered by their physician.  Exercises independent judgment under the general supervision of the Vice President of Patient Care Services.

Requirements:  Bachelor’s degree in course work approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association (ADA). Proof of passing the national examination administered by the Commission on Dietetic Registration (CDR). Participates in continuing educational requirements to maintain registration. Licensed dietitian, State of Vermont.  Minimum one (1) year experience with acute care or skilled care facility.

MT/MLT – Per Diem

Grace Cottage Hospital has an immediate opening for an MT/MLT in our full-service laboratory.  We serve both inpatients and outpatients and perform over 75,000 lab tests per year.   Graduate of an approved MT/MLT program; Experience in Blood Bank and Hematology preferred.  Strong computer skills and excellent customer service skills a must.  Position is per diem with fair share of weekends, holidays and after hours call.

HOUSEKEEPER – FULL TIME –32 Hours/Week,  Days (every other weekend and fair share of holidays. M-Th 8:30 am – 3:30; F 8:30 am – 3:00

Duties Include: maintaining the cleanliness of designated areas which include patient rooms, emergency rooms, cleaning assigned inpatient and outpatient areas in the facility, and performing other tasks as assigned by the housekeeping supervisor.

Requirements: Previous cleaning experience knowledge of infection control helpful; excellent customer service skills, ability to maintain patient privacy and confidentiality a must.

SCHEDULER

Job Summary: Answers telephones and speaks with patients to identify and meet their healthcare needs in a superior manner; performs call triaging and routes calls internally.  Schedules appointments  as requested by callers/patients by using  scheduling software; contacts patients to confirm or reschedule their appointment and uses clinic protocols to determine appropriate appointment assignment.  Documents patient calls, issues and resolutions appropriately.  Performs  new patient registration and sends welcome packets. Complete insurance verifications.

Requirements: High School graduate or equivalent; previous experience working in a clinical setting, knowledge of medical terminology, and/or experience with scheduling software is preferred.  Demonstrate the ability to properly use the phone and computer systems, fax  machines, photocopies, scanners, printers and other business machinery.  MUST have excellent customer service skills.  MUST maintain a cooperative professional working relationship with staff, outside agencies, community members and patients.  Maintain confidentiality of patient information according to Grace Cottage policy and federal/state guideline and regulations.

MEDICAL ASSISTANT

Job Summary: Facilitates patient flow throughout the encounter; completes chart prep, driven by visit type and chief complaint including reviewing the medical chart for current information regarding the patient’s medications, test results, hospital records, operative reports, notes from consulting specialists, etc.  Maintains and updates clinical information including identifying and documenting active problems, completing/resolving historical problems, updating social and family history, reviewing and updating allergy list, reconciling patient’s current medication list and determining if the patient needs refills after documenting any reported side effects of medications. Records chief complaint for visit, as obtained through patient interview.

Job Requirements: AAMA-CMA certification through the Certifying Board of the American Association of Medical Assistants (AAMA) required. Current BLS, electronic health record experience, computer skills and a strong team player.

LPN OFFICE NURSE

Job Responsibilities: Grace Cottage Family Health currently has an opening for a full-time LPN Office Nurse.  This position is responsible for the immediate care of patients; screening and assisting practitioners with exams and procedures.  Supports different medical providers when needed.  Works on quality improvement projects. Excellent communication skills with patients, families and staff to ensure quality care while maintaining confidentiality a must.  Current Vermont LPN license and a minimum of 3 years prior experience in an office setting required.  Position is Monday-Friday.

LICENSED NURSING ASSISTANT (LNA) – PART TIME – 24 Hours

Grace Cottage Family Health and Hospital has an immediate need for a part time LNA for our Inpatient Care unit.  Position is 24 hours per week on the evening shift.  Duties include assisting hospitalized patients with activities of daily living and providing basic nursing care as directed and supervised by a registered nurse. 

Requirements: Current Vermont licensure as a nursing assistant; minimum of 1 year experience preferred as an LNA in a Nursing  Home or Hospital setting; excellent communication and customer service skills.  Experience working with an electronic health record preferred.

UNIT SECRETARY – PER DIEM

Performs receptionist duties for the Inpatient Care Unit and Emergency Department (ED), greeting patients and visitors arriving at the Hospital. Alerts the ED RN of patients presenting for emergency care. Registers ED patients and new admissions to the Inpatient Unit following approved Hospital procedures, obtaining required information and consent for treatment. Functions as clerical support for the Inpatient Unit and ED.

Candidates must be detail oriented and able to multitask. Strong computer skills and excellent customer service skills a must. Knowledge of medical terminology a plus. Previous healthcare experience preferred.

FAMILY PRACTICE PHYSICIAN

Our Family Health Clinic is a provider based Rural Health Clinic including 11 providers covering family practice and behavior health with approximately 7,300 individual patients annually. Grace Cottage also operates a 19 bed critical access hospital, and a full-service retail pharmacy.

We currently are seeking a Family Practice Physician for our family health clinic. This position will provide a full scope of primary care services, including but not limited to diagnosis, treatment, coordination of care, preventative care and health maintenance to patients.

Family Practice Physician Profile:
• Outpatient Family Medicine
• Five physicians and three advanced practitioners, as well as, a Community Health Team.
• Charting: EMR …. Cerner
• May supervise PA or NP, and Nursing Staff assigned from pool
• Opportunity to include hospitalist rotation of one in seven.
• Optional ED Per Diem Shifts Available

Minimum Requirements: MD or DO degree from an accredited school of Medicine or Osteopathy; licensed to practice in Vermont; current DEA and DPS certificate of registration; Board Certified or eligible in Family Practice Medicine.

EMT / PARAMEDIC – Per Diem

 Job Summary:  Provides basic patient care, treatments and services within the EMT scope of practice that is appropriate for the patient’s condition and psychosocial/physical needs.

Duties: Clinical duties may include but are not limited to: preparing for procedure, preparing patients for exam, taking medical histories, collecting and testing (POC) of specimens, performing phlebotomy, starting peripheral intravenous lines, removing sutures, changing dressings, administration of meds (as outlined in scope of practice), performing EKGs and applying Zio Patches.  Will be responsible for registering emergency room patient and after hours out patients.

Requirements: HS graduate or GED; current BLS certification required; current and maintained Emergency Medical Technician Certification at the Basic, Advanced or Paramedic level; Obtain ACLS, PHTLS and PALS within 1 year of employment.

SPEECH LANGUAGE PATHOLOGIST – PER DIEM

Responsibilities: Responsible for administration and direct application of Speech/Language Pathology treatment modalities as prescribed by a referring physician. Evaluates plans, directs and treats all patients referred to speech/language pathology. Able to work in both inpatient and outpatient settings. Treatment experience with all age ranges helpful, but not required. Participates in performance improvement activities within the department and participates in CQI activities. Performs some administrative duties at the delegation of the Director of Rehabilitation Services. Participates in infection control, safety and educational programs, both in the department and on an organizational basis.

Requirements: Must have CCC-SLP. Holds a Master’s degree from an accredited college or school of speech/language pathology. Current State Licensure in speech pathology (or eligible). impact Current BCLS certification. New graduates welcome to apply.
