PATIENT CARE COORDINATOR – Full Time

 Job Summary: The Patient Care Coordinator initiates and transfers the referral process for patients; calls to make appointments; maintains spreadsheet of referral requests; supports providers and patients with referrals; and handles telephone calls. The patient care coordinator also provides support and coverage for registration and scheduling provider appointments.

Qualifications:

  • High School or equivalent
  • Computer skills, experience with data entry and word processing
  • Knowledge of medical terminology and healthcare insurance desirable
  • Must maintain confidentiality of patient information
  • Excellent customer service skills, strong team player

REGISTERED NURSE– Night Shift–Part Time

Job Summary: The Registered Nurse provides direct and indirect patient care as prescribed by the physician or attending mid-level provider, for all assigned patients the in-patient unit and/or emergency room.  Communicates with physicians, mid-levels, Nurse Manager and co-workers, as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology.

Qualifications: Graduate of an accredited nursing program. Minimum of 1-year experience preferred, however new graduates are encouraged to apply.

  • Current BLS certification

UNIT CLERK – Emergency Department/In-patient  — Per diem/Days

Shift:  7:00 am to 7:00 pm

Grace Cottage Family Health & Hospital has an immediate opening for a per diem Unit Clerk for the Emergency Department.

Job Responsibilities: Performs receptionist duties for the Inpatient Care Unit and Emergency Department (ED), greeting patients and visitors arriving at the Hospital.  Alerts the ED RN of patients presenting for emergency care.  Registers ED patients and new admissions to the Inpatient Unit following approved Hospital procedures, obtaining required information and consent for treatment.  Functions as clerical support for the Inpatient Unit and ED.

Requirements: Candidates must be detail oriented and able to multitask.  Strong computer skills and excellent customer service skills a must.  Knowledge of medical terminology a plus.  Previous healthcare experience preferred.

PHYSICAL THERAPIST – Full Time, 32 to 40 Hours/Week

Job Responsibilities: Treat a diverse outpatient caseload as a member of our rehabilitation team of physical and occupational therapists. This position will primarily serve general medical, orthopedic, and neurological conditions. We offer individual 45-minute sessions for all outpatient visits.

Job Requirements: Graduate of accredited program. Has current Vermont license in good standing (or willing to obtain).  The ideal candidate will have a strong orthopedic background. Excellent patient service skills. Strong team player.

MEDICAL LAB TECHNICIAN – Per Diem

Grace Cottage Hospital has an immediate opening for an MT/MLT in our full-service laboratory.  We serve both inpatients and outpatients and perform over 75,000 lab tests per year.   Graduate of an approved MT/MLT program; Experience in Blood Bank and Hematology preferred.  Strong computer skills and excellent customer service skills a must.  Position is per diem with fair share of weekends, holidays and after hours call.

SPEECH LANGUAGE PATHOLOGIST – Per Diem

Responsibilities: Responsible for administration and direct application of Speech/Language Pathology treatment modalities as prescribed by a referring physician. Evaluates plans, directs and treats all patients referred to speech/language pathology. Able to work in both inpatient and outpatient settings. Treatment experience with all age ranges helpful, but not required. Participates in performance improvement activities within the department and participates in CQI activities. Performs some administrative duties at the delegation of the Director of Rehabilitation Services. Participates in infection control, safety and educational programs, both in the department and on an organizational basis.

Requirements: Must have CCC-SLP. Holds a Master’s degree from an accredited college or school of speech/language pathology. Current State Licensure in speech pathology (or eligible). impact Current BCLS certification. New graduates welcome to apply.

CT/RADIOLOGY TECHNOLOGIST – Per Diem

Responsibilities: Provide exemplary service to both our internal and external customers. Employee will plan, organize, and accomplish activities in the Imaging Department according to hospital policies, standards of practice of the profession, and state and federal law. Possess the ability to work independently and as part of a team, providing high quality CT and Radiographic studies. Review physician orders and exercise great attention to detail. Actively participate in quality improvement projects within the department. Maintain confidentiality at all times and adhere to strict policies and procedures.

Requirements: Working knowledge of PACS; CT Certification; ARRT; VT State License; BLS certified; ability to collaborate with clinical support staff, physicians and other health care professionals as part of a team-based, patient-centered care approach.

LICENSED NURSING ASSISTANTS: Per Diem

Grace Cottage Hospital has an immediate need for per diem employees for our Inpatient Care unit. Positions can cover all shifts. Duties include assisting hospitalized patients with activities of daily living and providing basic nursing care as directed and supervised by a registered nurse.

Requirements: Current Vermont licensure as a nursing assistant; minimum of 1 year experience as an LNA in a Nursing Home or Hospital setting; excellent communication and customer service skills. Experience working with an electronic health record preferred. Successful candidate will be flexible in hours and shifts needed.
